Welcome to Chicago Westside Police and Youth baseball!

Practices will begin Tuesday, May 21, 2026. Regardless of the preferred park, practices will be on Tuesday and Thursday afternoons, and Saturday mornings.

Upon registration, each new player to the conference will be issued an equipment bag and all equipment each player will need. (Returning players may switch out their glove if they have outgrown it over the winter. New players to the conference WILL receive their own glove and helmet.) Any shared equipment will be sanitized after each practice.

Games are tentatively scheduled to begin June 13. Looking forward to a great season!

Medical Release - As the parent/legal guardian of the athlete I am registering for participation. I hereby authorize City of Refuge – Chicago, Chicago Westside and Southside Police and Youth Sports Baseball Program of Illinois, its agents, employees, volunteers and other officers to procure and consent to any medical emergency treatment, including hospital care, to be rendered to my child by or under the supervision of a licensed healthcare provider. The parent/legal guardian is responsible for any fees or costs. By choosing I accept it represents consent and agreement to the matters stated above. Waiver and Release In consideration of the risk of injury while participating in Baseball (the 'Activity') and as consideration for the right to participate in the Activity, I hereby, for myself, my heirs, executors, administrators, assigns, or personal representatives, knowingly and voluntarily enter into this waiver and release of liability and hereby waive any and all rights, claims or causes of action of any kind whatsoever arising out of my child or ward’s participation in the Activity, and do hereby release and forever discharge City of Refuge – Chicago, Chicago Westside and Southside Police and Youth Sports Baseball Program of Illinois, their affiliates, managers, members, agents, attorneys, staff, volunteers, heirs, representatives, predecessors, successors and assigns, for any physical or psychological injury, including but not limited to illness, paralysis, death, damages, economical or emotional loss, that my child or ward may suffer as a direct result of my child or ward’s participation in the aforementioned Activity, including traveling to and from an event related to this Activity. I am having my child or ward voluntarily participate in the aforementioned Activity entirely at my own risk. I am aware of the risks associated with traveling to and from as well as my child or ward participating in this Activity, which may include, but are not limited to, physical or psychological injury, pain, suffering, illness, disfigurement, temporary or permanent disability (including paralysis), economic or emotional loss, and death. I understand that these injuries or outcomes may arise from my child or ward’s own or others’ negligence, conditions related to travel, the condition of the Activity locations(s) and the activity itself. Nonetheless, I assume all related risks, both known or unknown to me, of my child or ward’s participation in this Activity, including travel to, from and during this Activity. I as the parent or guardian of the participant agree to indemnify and hold harmless City of Refuge – Chicago, Chicago Westside and Southside Police and Youth Sports of Illinois against any and all claims, suits or actions of any kind whatsoever for liability, damages, compensation or otherwise brought by the participant or parent and next friend or guardian of the participant including attorney’s fees and any related costs, if litigation arises pursuant to any claims made by the participant or parent and next friend or guardian of the participant. If City of Refuge – Chicago, Chicago Westside and Southside Police and Youth Sports of Illinois incurs any of these types of expenses, I agree to reimburse City of Refuge – Chicago, Chicago Westside and Southside Police and Youth Sports of Illinois. I acknowledge that City of Refuge – Chicago, Chicago Westside and Southside Police and Youth Sports of Illinois and their directors, officers, volunteers, representatives and agents are not responsible for errors, omissions, acts or failures to act of any party or entity conducting a specific event or activity on behalf of the City of Refuge – Chicago, Chicago Westside and Southside Police and Youth Sports of Illinois. I acknowledge that this Activity may involve a test of a person’s physical and mental limits and may carry with it the potential for death or serious injury. The risks may include, but are not limited to, those caused by facilities, temperature, weather, lack of hydration, condition of participants, equipment, vehicular traffic and actions of others, including but not limited to, participants, volunteers, spectators, coaches, referees and tournament officials. I acknowledge that I have carefully read this 'Waiver and Release' and fully understand that it is a release of liability. I expressly agree to release and discharge City of Refuge – Chicago, Chicago Westside and Southside Police and Youth Sports of Illinois and all of its affiliates, managers, members, agents, attorneys, staff, volunteers, heirs, representatives, predecessors, successors and assigns, from any and all claims or causes of action I may have and my child or ward may have and agree to voluntarily give up or waive any right that my child or ward otherwise have to bring a legal action against City of Refuge – Chicago, Chicago Westside and Southside Police and Youth Sports of Illinois for personal injury or property damage. To the extent that statute or case law does not prohibit releases for negligence, this release is also for negligence on the part of City of Refuge – Chicago, Chicago Westside and Southside Police and Youth Sports of Illinois, its agents, and employees. In the event that my child or ward should require medical care or treatment, I agree to be financially responsible for any costs incurred as a result of such treatment. I am aware and understand that I should carry my own health insurance. In the event that any damage to equipment or facilities occurs as a result of my or my family’s willful actions, neglect or recklessness, I acknowledge and agree to be held liable for any and all costs associated with any actions of neglect or recklessness. This Agreement was entered into at arm’s-length, without duress or coercion, and is to be interpreted as an agreement between two parties of equal bargaining strength. Both the Parent or Guardian of the Participant, and City of Refuge – Chicago, Chicago Westside and Southside Police and Youth Sports of Illinois agree that this Agreement is clear and unambiguous as to its terms, and that no other evidence will he used or admitted to alter or explain the terms of this Agreement, but that it will be interpreted based on the language in accordance with the purposes for which it is entered into. In the event that any provision contained within this Release of Liability shall be deemed to be severable or invalid, or if any term, condition, phrase or portion of this agreement shall be determined to be unlawful or otherwise unenforceable, the remainder of this agreement shall remain in full force and effect, so long as the clause severed does not affect the intent of the patties. If a court should find that any provision of this agreement is invalid or unenforceable but that by limiting said provision it would become valid and enforceable, then said provision shall be deemed to be written, construed and enforced as so limited. I also give the City of Refuge – Chicago, Chicago Westside and Southside Police and Youth Sports of Illinois, its coaches or volunteers’ the right to copyright and/or publish, reproduce, or otherwise use my child’s name, voice, and likeness and/or photographs, and audiovisual recordings that include my child for instruction, advertising, program website publications, brochures or any other lawful purpose whatsoever. I hereby authorize and give my full legal consent for City of Refuge – Chicago, Chicago Westside and Southside Police and Youth Sports of Illinois to apply for membership to the AAU on behalf my child. I understand that this consent for application includes agreeing to comply with all the provisions of the AAU Code, including its constitution, bylaws, policies, procedures, regulations and rules, I hereby agree to relinquish all rights, title and interest I may have in the finished product and waive all rights to any compensation thereof. * *

(1) I/We, the parents/guardians of the above-named candidate for a position on a Little League team, hereby give my/our approval to participate in any and all Little League activities, including transportation to and from the activities. (2) I/We know that participation in baseball or softball may result in serious injuries and protective equipment does not prevent all injuries to players, and do hereby waive, release, absolve, indemnify, and agree to hold harmless the local Little League, Little League Baseball, Incorporated, the organizers, sponsors, supervisors, participants, and persons transporting my/our child to and from activities from any claim arising out of any injury to my/our child whether the result of negligence or for any other cause. (3) If applicable, I/We agree to return upon request the uniform and other equipment issued to my/our child in as good conditions as when received except for normal wear and tear. (4) I/We agree to provide proof of legal residence or school enrollment (as defined by Little League Baseball, Incorporated at LittleLeague.org/residence) and age. I/We understand that our child (candidate) must be eligible under the residence/school attendance and age regulations of Little League Baseball, Incorporated, to participate in this Local League, and that if any controversy arises regarding residence/school attendance and/or age, the decision of the Little League International Charter Committee in Williamsport, Pennsylvania shall be final and binding. I/We further understand that if any participant on a Little League team does not qualify for participation in the league based on residence (as defined by Little League Baseball, Incorporated) and/or age, such participant and/or team on which he/she participates be found ineligible, and forfeit(s) and/or suspension of Tournament privileges may be decreed by action of the Little League International Charter Committee or Little League International Tournament Committee. (5) I/We agree that our child (candidate) may be required to try out for a team. If such does not attend at least 50 percent of the tryouts, local Board-of -Directors' approval is required for such candidate to be placed on a team. (6) If applicable, I/We understand that our child (candidate) may be chosen at any time to play on a Major Division team, if he or she is of the correct age for such division as determined by the local league and Little League Baseball. Declining to move up to such Major Division team will result in forfeiture of eligibility for the Major Division for the current season, and may be subject to further restrictions by the local league. (7) I/We will furnish a certified birth certificate of the above-named candidate to League Officials if requested. (8) I/We understand that my information as the parent or guardian of such above-named candidate is sent by the local league to Little League International each year. Such use of information by Little League International can be found here: www.LittleLeague.org/privacypolicy. You may opt-out of communications from Little League International at any time.
